COPD and GOLD stage I.

چکیده
An abnormally low FEV 1 /FVC ratio is universally accepted as indicative of obstructive lung disease. Clearly, the choice of cutoff to defi ne abnormality is important. The GOLD (Global Initiative for Chronic Obstructive Lung Disease) group set the lower limit of normal (LLN) at 0.7 irrespective of age, defi ning GOLD COPD stage I as FEV 1 /FVC below 0.7 and FEV 1 above 80% predicted. 1 In a recent issue of CHEST (January 2012), Mannino and Diaz-Guzman 2 argue that patients in GOLD stage I are at increased risk of premature death from respiratory causes. As shown in many publications, the 0.7 cutoff is too simplistic. After age 45, the LLN based on the fi fth centile of the FEV 1 /FVC ratio falls progressively below 0.7, meaning that the 0.7 cutoff identifi es many older patients above the LLN as false positives. Several studies have tried to validate GOLD stage I for identifying obstructive lung disease:

The Lipid Profile Parameter in Chronic Obstructive Pulmonary Disease Patients and Correlation with Severity of Disease
Introduction: More than 90% of the deaths caused by chronic obstructive pulmonary disease (COPD) occur in the low- and middle -income countries. The main aim of this study was to investigate the lipid profile levels in COPD patients and examine the correlation of total cholesterol, triglycerides, low-density lipoproteins, high density lipoproteins, and LDL/HDL risk ratio with COPD stages that a...
متن کاملStereotactic body radiotherapy for Stage I lung cancer with chronic obstructive pulmonary disease: special reference to survival and radiation-induced pneumonitis
This retrospective study aimed to evaluate radiation-induced pneumonitis (RIP) and a related condition that we define in this report--prolonged minimal RIP (pmRIP)--after stereotactic body radiotherapy (SBRT) for Stage I primary lung cancer in patients with chronic obstructive pulmonary disease (COPD). We assessed 136 Stage I lung cancer patients with COPD who underwent SBRT. Airflow limitation...
متن کاملAn international survey of chronic obstructive pulmonary disease in young adults according to GOLD stages.
BACKGROUND The recently published GOLD guidelines provide a new system for staging chronic obstructive pulmonary disease (COPD) from mild (stage I) to very severe (stage IV) and introduce a stage 0 (chronic cough and phlegm without airflow obstruction) that includes subjects "at risk" of developing the disease. METHODS In order to assess the prevalence of GOLD stages of COPD in high income co...
متن کاملThe pharmacological cost of COPD during Greek economic crisis
INTRODUCTION The economic crisis in Greece has substantially affected patients with COPD. The reduction of disposable income has its consequences on patients' ability to afford their medication. The aim of the study is to evaluate the cost of treatment for patients with COPD and the influence of the financial crisis to the patients. METHODS Data were collected from 189 patients (male: 178, me...
متن کاملResting heart rate is a predictor of mortality in COPD.
The clinical significance of high heart rate in chronic obstructive pulmonary disease (COPD) is unexplored. We investigated the association between resting heart rate, pulmonary function, and prognosis in subjects with COPD. 16 696 subjects aged ≥40 years from the Copenhagen City Heart Study, a prospective study of the general population, were followed for 35.3 years, 10 986 deaths occurred. An...
